Prepare for your Live Scan
Obtain the correct Request for Live Scan Service form BCIA 8016, from the agency or employer requesting your fingerprints.

Completely fill out form BCIA 8016 with the correct ORI number. We can not complete your Live Scan service otherwise.

Forms may be available online, check with your requesting agency.
A valid, unexpired government-issued photo ID (ex: Driver’s License, Passport, State ID) is required to complete a Live Scan fingerprint service.​
The cost of a Live Scan service is the fingerprint rolling fee plus government fees.

Our Live Scan Rolling Fee: $25

Standard government fingerprint processing fees:
DOJ fee: $32
FBI fee: $17
Ex: Rolling Fee + Government Fees = Total Cost ($25+$32+$17=$74)
